Community Action Wayne/Medina Housing programs are for low-income homeowners and renters unless otherwise noted. Each program is based on funding availability and frequently has a waitlist. When possible, multiple programs are used in combination on a home in order to achieve the most cost savings and help the largest number of households in both counties. At no cost, households can receive help with energy efficiency, weatherization, and/or health and safety home repair.
Street Outreach Team (Rural Outreach)
Provides on-the-ground support to individuals experiencing homelessness in rural areas by connecting them with shelter, resources, and essential services to help stabilize their situation.
Homeless Crisis Response Program
Provides short-term and medium-term housing assistance with supportive services to help individuals experiencing homelessness or those at risk of homelessness achieve housing stability.
Permanent Supportive Housing
Combines long-term housing assistance with supportive services to help individuals with disabilities and chronic homelessness achieve housing stability and independence.
Supportive Housing Program
Provides temporary or transitional housing along with case management and supportive services to help individuals and families move toward permanent housing solutions. Individuals must be chronically homeless and have a disability.
Note: For all programs, individuals and families with the longest length of homelessness and highest level of need are prioritized for services. Some services have a waiting list.
