ATTENTION E-HEAP CLIENTS
(please read carefully)
You must have all of the following information with you at the time of intake.
- You need BOTH utility bills with you even if you are only getting assistance with one utility.
- If you need fuel oil/propane, you must have LESS than a 10 day supply.
- For GAS/ELECTRIC, you MUST have a DISCONNECT with you.
- Your last THREE (3) months proof of income for everyone in the home.
- If you get paid weekly, you will need your last 13 pay stubs.
- If you get paid bi-weekly, you will need your last 6 pay stubs.
- If you do not have all of your pay stubs, you will need to get a statement from your employer.
- If your income is OWF, Social Security or SSI, you will need a printout of your monthly amount.
- If you receive child support, you will need a printout of your monthly amount.
- You must have a Social Security numbers for everyone in the home.
- We must have your landlords name, address and phone number.
- If needing a deposit paid on your utility bill, we must have a copy of your lease.
- If you have been disconnected for more than 10 days, you need a pending account number from the utility company. Contact the utility company for that information before you see the intake worker.
